Appendix to application for a residence permit to visit Sweden or live with someone in Sweden.
Use this form if you plan to operate as a guest artist in Sweden. By guest artist we mean writers, journalists, artists, musicians, filmmakers and others who in their home countries have been subjected to persecution and who are now invited by Swedish municipalities, county councils and regions to have a safe haven to live and work in Sweden for some time.
Fill in this form as parent or legal guardian when you apply for an alien's passport, an emergency alien's passport or a travel document for a child under the age of 18.
Use this form to apply for a permit for a child under the age of 18 who is to be, or has been, adopted, and where the adoption is not negotiated by an authorized Swedish adoption organisation and/or under the Hague Convention.
For a person who wants to give his or her consent to a DNA analysis, as a part of the investigation in a case of residence permit.

Use this form if you want to apply for a permit to visit Sweden for more than 90 days. You should also use this form if you want to extend your on-going visit in Sweden and the total length of the visit would then be longer than 90 days.
Appendix to the application for a permit for an on-going visit via the e-service from Sweden. This form is to be used by a person or organisation who is guaranteeing the means of support for a person applying via the e-service for a permit for an on-going visit in Sweden. You must use this form if the total length of the visit will then become longer than 90 days.
